Community Case Management

Under this program, Pathways will provide a certified, fully trained Case Manager for any qualified consumer. This Case Manager will assist and support the consumer in planning his/her life, scheduling meetings and monitoring results. Each consumer is fully assured the opportunity to participate fully in the personal planning process.

The goal of the Case Manager is to focus on the consumer and help them to reach their vision of their personal desired life style. Monitoring of the support system is done through personal contacts with the consumer, family and involved support staff. The Community Case Manager is always monitoring physical, emotional, and social well being, and communication with family, peers, and staff to assure that the consumers needs in the workplace and home environment are fulfilled.
